* Food and beverage prices in the menu

* Comprehensive general liability insurance for guests in the amount of:

o For deluxe and first-class hotels - P 100, 000

o For standard and economy-class hotels - 50, 000

* Payment of CTAO hotel registration fees

o Deluxe - P 11,000

o First-class - 9,000

o Standard - 6,000

o Economy - 4,000

2. Apartelle/Tourist Inn/Pension House

* DTI registration (for single proprietorships)

* SEC registration (for corporations)

* Barangay clearance

* BPLO application form with clearance

* Contract of lease (if business space is rented)

* Annual general liability insurance for guests in the amount of not less than P 10,000.00

* Payment of CTAO apartelle registration fees

o With 75 or less rooms - P 1,200

o With 76 to 100 rooms - 2,400

o With more than 100 rooms - 4,200

3. Travel Agency

* DTI registration (for single proprietorships)

* Surety Bond of P 500,000 in favor of the QC Government (renewable yearly)

4. Tourist Transport Operator

* DTI registration (for single proprietorships)

* Surety Bond of P 10,000

* LTO Registration

* LTFRB Certificate of Public Conveyance Franchise or Authorization

* Transportation Rates approved by LTFRB

* Motor Vehicle Liability Insurance

o Third-party liability - P 20,000 per unit

o Passenger accident - P 12,000 per person

* Picture of Vehicle/s showing the side, back, and front views with the company's name and logo imprinted at its rear and sides

* Proof of Ownership or lease of an area that serves as maintenance depot and garage

* List of officials and employees and their respective designations, nationalities, home addresses, which is certified correct under oath by the Secretary of the firm or the proprietor himself.

* Payment of CTAO Registration fee of P 1,000

5. Tour Guide

* Certificate of Seminar on Tour Guiding from the Department of Tourism

* NBI Clearance
