Declaration/Registration of Newly Assessed Bulidings/ Condominiums and Machinery; request for Re-assessments (Form 103) a) Approved building permit/building plan( actual measurement if plan is not available)b) Certicate of occupancy or building permit(optional); c) Photocopy of Title or tax declaration af lot where the structure is erected/located; d) Sworn statement of owner declaring the property e) Individual condominium certificate of title (for condominium and townhouses) f) Approved plan and master deed(fo condominium) g) SEC Registration and certification of actual use (for exempt properties) h) For machineries,list of machineries,sworn statement of owner/s and SEC Registration in case of registration of commercial/industrial machineries) i) Picture property (3"x5" colored)-- note: if vacant lot, include portion of the sidewalk or if with imprrovement, picture should be front view
