Steps for Securing an Occupational Permit with the Business Permits and Licensing Office

Basis:
Article 22 of the Quezon City Revenue Code as amended:

Section 83. It shall be unlawful for any person to exercise his occupation or calling within the jurisdiction of the city ,without acquiring the individual Mayor's Permit..

Section 84. The Mayor's Permit fee on occupation or calling shall be paid to the city Treasurer before any occupation or calling can be lawfully pursued in the city."

Tips for faster processing:

  • Start your application early so that you can complete the entire process in one day.

  •                - Alternate Form for multiple applicants
  • Choose to have your sputum/stool exam at any of the 86 accredited laboratories.
  • Ask your company to get your Occupational Permit Application Forms for all the employees.
  • For those renewing their permits, the best month to do so is December, when there is very little queuing for these permits.

1. Get your HEALTH CARD from the City Health Department

Requirements for first time applicants:

a) Get your Order of Payment and pay Php 194.00 for your Helth Card at the City Treasurer's cashier.

b) Submit for laboratory examination, your stool and sputum specimen and get the results. The stool and sputum specimen can be submitted to the City Health Department or to any accredited laboratories (please see listing)

c) Attend an HIV/AIDS seminar
   Schedule yourself in any of the 5 sessions held everyday, five days a week

d) Get your picture taken at the Health Department for your Health Card

e) Get your Health Card

Requirements for renewal of Health Card:

a) Get your Order of Payment and pay Php 144.00 for your Helth Card at the City Treasurer's cashier.

b) Submit for laboratory examination, your stool and sputum specimen and get the results. The stool and sputum specimen can be submitted to the City Health Department or to any accredited laboratories (please see listing)

c) Bring your old Health Card for stickering at the City Health Department (your Health Card is good for 3 years)

2. Get your OCCUPATIONAL PERMIT CARD from the business Permits and Licensing Office (BPLO)

a) Go to the BPLO and show proof of payment of Health Card (show your Health Card; if it is still being processed, you can show your receipt or Order of Payment for the Health Card)

b) Submit your Community Tax Certificate (cedula) and NBI or Police Clearance

c) Get your Order of Payment for the Occupational Permit Card and pay at the City Treasurer's cashier. (The fee is Php 95.00 for all occupations except for Guest Relations Officers, who pay Php 170.00)

d) Have your picture taken at BPLO for your Occupational Permit Card.

e) Pick up your card.

Additional requirements for employees below 18 years old:
a. Parental consent
b. Birth certificate/baptismal certificate
c. Valid ID of parent/giving consent

Special Service

Companies can apply for the Health Card and Occupational Permit Card for all their employees altogether

Companies can request to:

  • Have their employees' stool and sputum examination done at any accredited laboratories (please see listing).
  • Have their HIV/AIDS seminar done for their employees at their company premises.
  • The City Health Department will send the lecturer(s) at the agreed upon schedule.
  • Submit their employees' photos for their Health Card. If the photo-taking is not done at the Health Department, the ID provided will be a "white" Health Card, and not a self-laminated card This card is good only for one year, while the self-laminated card has provisions for 3 years use.

2. A company can pick up application forms for the Occupational Permit Card from the BPLO, for all their employees. A company official can file these application forms as a group, after paying the required fees and providing the needed NBI or Police Clearances.
